By registering with CLYDE/ N.M. School of Dance I ackowledge that the registration fee must be paid to process my childs registration. I also acknowledge that registration fees and tuition are non-refundable.

If parent selects "no" this application will not be processed.

Are you able to commit to bringing your child to class consistently, on time, and prepared ?

If your child is absent consistently parent/guardian will receive one warning before the student forfeits their space.
